Furthermore, I understand that the actions taken in this program will not guarantee my child’s safety in a motor vehicle crash. In consideration of my participation in the car seat clinic, I (and my child, if I am signing as parent or guardian) release Children's Wisconsin, Inc. and its affiliates, officers, directors, employees, agents and volunteers (“Children’s”) from any and all liability or claim for loss, injury or illness that I (or my child) may sustain related to this event. I understand that this release applies to myself (or my child) and my (or my child's) personal representatives, heirs and assigns, and that this release excludes any harm or loss caused intentionally or recklessly by Children’s. I understand the risks related to improperly installing a car seat may include injury and/or death. I (and my child, if I am signing as parent or guardian) also waive the right I (or my child) have to bargain for different release of liability terms. By signing my name below, I understand that I am signing this document and I agree to the terms contained herein.
