This form is for requesting Agency Head or an Admin account for your department/agency. Agency Head's are able to run reports and see the dashboad for all checks done through their agency. You can have multiple Agency Heads for your department. Admin accounts are for co-workers that are not certified through Safe Kids and want access to help submit forms into the system or become the Agency Head.
Both permissions to the account will last a year. After the year is up - the account's permissions will be removed or blocked and new form will need to be filled out/signed for.
