I understand and acknowledge that the child restraint must at all times be used in accordance with the instructions provided by the manufacturer of the child restraint and the instructions provided by the vehicle manufacturer. I also understand that if I do not use the child restraint as advised in the manufacturers' instructions, the child restraint may not be effective in a vehicle collision.

By signing below, I indicate that I have read this form, understand it, and agree to accept any and all responsibility for any misuse of the child restraint device or any installation contrary to the manufacturer's instructions. I also agree to release ICJI, their representatives, affiliated inspection sites, affiliated car seat clinic sponsors, and anyone for whom they may be legally responsible from any present or future liability for any injuries or damages that may result from a vehicle collision or otherwise.

I agree that if I received a child restraint free of charge that I will not sell the child restraint. I further acknowledge that the child restraint was provided from state or federal funding and that I am subject to penalties including but not limited to, reimbursement for the cost of the child restraint, if I sell the seat to any third party.

I understand and agree that the sole purpose of this program is to help reduce the incidence of improper installation and use of car seats, booster seats and seat belts,
and that this inspection and demonstration is being provided as a free educational service to me. I realize that the program sponsors and certified child passenger safety
technicians inspecting the seat(s) cannot fully evaluate the quality, safety, or condition of my car seat, booster seat, or the vehicle seat, safety belts, or any component of
the vehicle now or in the future. Furthermore, I understand that the actions taken in this program will not guarantee my child’s safety in a motor vehicle crash. I
understand that it is important to read and follow the instruction manuals for both the vehicle and the car seat. For these reasons, I hereby release any program
participants, any participating organizations or individuals, including the site owner, from any present or future liability for any injuries or damages that may result
from a vehicle collision or otherwise. I understand and agree that I will be the final person performing the installation of the seat and securing my child in the vehicle.

The Indiana Criminal Justice Institute (“ICJI”) works with local and community organizations to increase public awareness on the proper use of child restraint devices, and may at times distribute such safety devices. Neither ICJI, nor its affiliates, such as permanent child safety seat inspection stations, are merchants or sellers of goods, and make no warranties, expressed or implied, about the safety of these devices.

I understand and acknowledge that the child restraint must at all times be used in accordance with the instructions provided by the manufacturer of the child restraint and the instructions provided by the vehicle manufacturer. I also understand that if I do not use the child restraint as advised in the manufacturers' instructions, the child restraint may not be effective in a vehicle collision.

By signing below, I indicate that I have read this form, understand it, and agree to accept any and all responsibility for any misuse of the child restraint device or any installation contrary to the manufacturer's instructions. I also agree to release ICJI, their representatives, affiliated inspection sites, affiliated car seat clinic sponsors, and anyone for whom they may be legally responsible from any present or future liability for any injuries or damages that may result from a vehicle collision or otherwise.

I agree that if I received a child restraint free of charge that I will not sell the child restraint. I further acknowledge that the child restraint was provided from state or federal funding and that I am subject to penalties including but not limited to, reimbursement for the cost of the child restraint, if I sell the seat to any third party.
