I understand and agree that the sole purpose of this child safety seat inspection checklist program is to help reduce the incidence of the improper installation of child safety seats in motor vehicles; that this inspection is being provided as a free service to me; that this program cannot fully evaluate the quality, safety, or condition of the car safety seat, the car safety seat provided or any component of my vehicle, including the seats, safety belts or anchors; and this program cannot guarantee the safety of any child occupying the car safety seat in the even of a vehicle collision. However, I understand that a properly used child safety seat may reduce fatal injury by up to 71% for infants and 54% for toddlers and that it is important that any operator of the vehicle read both the vehicle and car seat instruction manuals in order to maximize the safety of the occupant of the child safety seat in the event of a vehicle collision.

For these reasons, I hereby release Oneida County, the Oneida County Health Department, Oneida County elected and appointed officials and employees and any program participants from any present and future liability for any injuries or dangers that may result from a vehicle collision or otherwise.
